Frequently Asked Questions
Mindfulness Counseling
I meet with clients either on a weekly or biweekly basis typically. The duration of therapy depends on the consistency of attendance and progress on the goals you would like to work on.
Mindfulness Counseling is located in Olde Town Arvada at 5738 Olde Wadsworth Blvd. Arvada, CO 80033 in the Town Square Professional Building.
Yes! I understand sometimes it may be more accessible to meet virtually. However, I do require all virtual meetings to be held from a quiet and confidential space to ensure you can get the most of your session.
- I typically start with a virtual 20 minute consultation to touch base about what you would
like to work on and ensure we are a good fit for each other. You can reach out to schedule a
consultation by filling out the ‘contact me’ form on the website.
I currently accept Aetna, United, and Cigna. I also accept the Employee Assistant Programs
of Lyra and Curalinc.
No, I do not accept insurance for couples counseling. I can provide services as an out of network provider and you are able to go through your out of network benefits for possible reimbursement.
If you have out of network benefits, I will provide you with a ‘superbill’ (a specialized invoice that can be submitted to insurance companies for reimbursement) to submit to your insurance company. You will be responsible for paying for sessions at the time of service and then, at the start of each month, I will provide you with a superbill of all sessions for the prior month. You can then submit this to your insurance company for reimbursement. It is not guaranteed your insurance company will provide reimbursement, because it is dependent upon your health insurance plan and policies.
If you are unfamiliar with your out of network benefits I recommend reaching out to your insurance and asking these questions below:
- Are mental/behavioral health services (Current Procedural Terminology/CPT codes 90837
for individual therapy, or 90847 for couples therapy) covered by my out-of-network
benefits? - What is my out-of-network deductible?
- How much of my out-of-network deductible has already been met
- How much of the fee is reimbursed for out-of-network providers?
- How do I submit for reimbursement?
- How long do I have to submit my Superbill?
- How long will it take to be reimbursed for sessions after I have submitted my claims?
Individual Sessions:
- $175 – 60 minute Intake Session
- $150 – 50 minute Follow Up Session
Couples Counseling:
- $200 – 60 minute Intake Session
- $180 – 60 minute Follow Up Session
- $200 – 90 minute Follow Up Session
I accept discover, visa, mastercard, and HSA.
A Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created and is subject to change.
The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill.
The Good Faith Estimate is not a contract and does not require you to obtain the services from the provider identified on the Good Faith Estimate. If you are billed for more than your Good Faith Estimate, you have the right to dispute the bill.
You may contact the health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.
You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill.
There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on this Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.
To learn more and get a form to start the process, go to www.cms.gov nosurprises or call HHS at (800) 368-1019.
For questions or more information about your right to a Good Faith Estimate or the dispute process, visit www.cms.gov/nosurprises or call (800) 368-1019.
Keep a copy of your Good Faith Estimate in a safe place or take pictures of it. You may need it if you are billed a higher amount.
